About this agreement
This agreement covers the Action Humanitarian Philippines volunteer trip from November 5-15, 2026. "Participant" means the adult or minor volunteer named in this form. "Released Parties" means Action Humanitarian, its affiliated organizations, directors, officers, employees, volunteers, trip leaders, agents, contractors, donors, sponsors, local partner organizations, host-property owners, and transportation or activity providers involved with the trip, together with each of their respective representatives, successors, and assigns. "I" and "me" mean the adult Participant or, for a minor Participant, the parent or legal guardian signing this agreement.
Trip activities
The trip includes international and domestic flights, travel by van and boat, lodging, meals, volunteer projects, construction or cleanup work, lifting, use of hand or power tools, walking on uneven or unfinished surfaces, swimming, snorkeling, beach activities, excursions, and related group activities. Airlines, transportation companies, lodging providers, guides, medical providers, and other vendors may be independent businesses that Action Humanitarian does not control. Schedules and activities may change when travel, weather, safety, health, or local conditions require it.
Acknowledgment of risk
I understand that international travel, volunteer work, and water activities carry risks that cannot be completely eliminated. These include transportation accidents, falls, cuts, strains, tool or equipment injuries, swimming or snorkeling injuries, drowning, food- or water-related illness, infectious disease, changing weather or worksite conditions, theft or property loss, and medical care or evacuation that may be delayed or different from care available at home. Serious injury or death is possible. I voluntarily accept and assume the risks that reasonably come with the trip and its activities for myself and, when signing for a minor, in deciding to allow the minor to participate.
Health, safety, and conduct
I am responsible for deciding whether the Participant is physically and medically able to take part and for the Participant's passport, medications, health precautions, and other travel requirements. I will tell a trip leader about any condition or limitation that may affect safe participation or emergency care. The Participant will follow trip-leader instructions, use required safety equipment, remain with the group when directed, respect local laws and customs, and avoid unsafe, illegal, harassing, or disruptive conduct. Action Humanitarian may limit or end participation when reasonably necessary for safety, legal compliance, the group, or the community. I am responsible for additional travel or lodging costs caused by the Participant's misconduct or refusal to follow instructions.
Medical care, insurance, and personal property
If the Participant cannot make or communicate a medical decision, I authorize trip leaders to obtain or arrange medical care, transportation, rescue, or evacuation they reasonably believe is appropriate. I understand that care may be limited or delayed and cannot be guaranteed. I am responsible for related costs and for confirming that the Participant has adequate health, travel, and evacuation insurance. I am also responsible for the Participant's passport, money, luggage, electronics, and other personal property.
Adult release
In exchange for being allowed to participate, I release the Released Parties from claims for injury, illness, death, property damage, or other loss arising from the trip or its activities, including claims caused in whole or in part by the ordinary negligence of a Released Party, to the fullest extent allowed by law. I agree not to sue on a claim covered by this release. This release does not cover gross negligence, reckless conduct, willful or intentional misconduct, or any liability that cannot legally be released.
Responsibility for my conduct
I agree to indemnify and hold the Released Parties harmless from third-party claims and reasonable costs caused by my negligent, reckless, or intentional conduct, my violation of law or safety instructions, or my material breach of this agreement. I am not responsible under this paragraph for a Released Party's gross negligence, reckless conduct, willful misconduct, or other liability that cannot legally be shifted to me.
Legal terms and electronic signature
Utah law governs this agreement, except where another law must apply. Any claim that may lawfully be heard in Utah will be brought in a court with jurisdiction in Utah. If one part of this agreement is unenforceable, the remaining parts will continue in effect and the affected provision will be limited only as much as necessary. This agreement contains the complete understanding about these matters. I have read it, had the opportunity to ask questions, and sign voluntarily. I agree that my electronic signature has the same effect as my handwritten signature.
Certification
I certify that I will be at least 18 years old on November 5, 2026. I have read and understand this agreement and voluntarily agree to its assumption of risk, release, responsibility, medical authorization, and other terms.
